Your First Visit

Here are some things you may find helpful before your first visit:

1. Schedule your appointment. Contact our office during business hours to find a time that works for your family.

2. Complete the initial forms. Our office will let you know what forms must be completed for your appointment. These forms are available on our website here and can be completed in advance or completed in person during your first visit.

3. Prepare any questions you may have about your child or our practice.

4. We’re located at 8651 W. North Avenue. You may park in our private lot west of the main building.
